Mr David Iles

Mr Iles is a Consultant Subspecialist Urogynaecologist and Clinical Lead of the Warrell Unit at St Mary’s Hospital, Manchester University NHS Foundation Trust, one of the largest tertiary urogynaecology referral centres in the United Kingdom. He has more than twenty years’ experience in obstetrics and gynaecology, the last decade focused on subspecialty practice in urogynaecology and pelvic floor surgery. He holds full subspecialty accreditation in urogynaecology, awarded on completion of formal subspecialty training, and his NHS practice is devoted entirely to the assessment and surgical management of women with pelvic floor disorders.

The conditions Mr Iles assesses and treats include pelvic organ prolapse of all compartments, urinary stress incontinence, overactive bladder and urgency incontinence, voiding dysfunction, recurrent urinary tract infection, bladder pain syndrome, urogenital fistula, and obstetric anal sphincter injury and other pelvic floor trauma sustained during childbirth. He has a particular specialist interest in mesh-related complications, and is a key member of the St Mary’s Mesh Complications Service – a designated regional centre and one of the largest such services in the country – where he assesses and manages women referred with complications of both incontinence and prolapse mesh from across the North of England and beyond.

The procedures Mr Iles performs include vaginal and abdominal prolapse repair (including sacrocolpopexy and sacrospinous fixation), continence surgery including colposuspension and autologous fascial sling, and the full range of mesh-removal and revision procedures, including complete and partial excision of mid-urethral and transvaginal mesh. He has particular expertise in minimal access (laparoscopic) and robotic-assisted pelvic floor surgery, including laparoscopic and robotic sacrocolpopexy. He undertakes complex revision surgery for women with adverse outcomes following previous pelvic floor procedures, and is routinely involved in the review and management of high-risk and previously operated cases.

Mr Iles’ clinical interests centre on the surgical management of pelvic‑organ prolapse, with a strong emphasis on minimally invasive (laparoscopic and robotic) techniques that aim to reduce morbidity while preserving functional outcomes. He is actively involved in advancing mesh‑related care, both by refining removal and revision strategies for incontinence and prolapse mesh and by contributing to institutional protocols that mitigate complications.

His focus also extends to the management of urinary incontinence, ranging from open and robotic-assisted colposuspension to the refinement and application of autologous fascial sling techniques. Recognising the long-term impact of childbirth trauma, Mr Iles dedicates a substantial portion of his practice to obstetric anal sphincter injury and other pelvic floor injuries sustained during delivery, integrating surgical repair with multidisciplinary rehabilitation. He places particular emphasis on ensuring that rigorous consent processes and transparent peri-operative decision-making are consistently upheld, so that women are fully informed about the benefits, risks and alternatives of each intervention

As Clinical Lead for Urogynaecology at the Warrell Unit, St Mary’s Hospital, Mr Iles is responsible for service development, clinical governance, and the design and delivery of multidisciplinary care pathways for women with pelvic floor disorders. He leads one of the largest tertiary urogynaecology and mesh complications services in the United Kingdom. He is also Undergraduate Lead for Gynaecology at St Mary’s Hospital.

Mr Iles was awarded an MD by the University of Manchester for research in urogynaecology, undertaken during a period as Clinical Research Fellow at St Mary’s Hospital. His research interests include surgical outcomes following pelvic floor surgery, patient-reported outcome measures and body image in urogynaecology, the management and outcomes of mesh-related complications, and the consequences of obstetric anal sphincter injury.

Mr Iles has authored peer-reviewed publications in the field of urogynaecology and pelvic floor disorders, including work on surgical outcomes following mesh excision, patient-reported outcome measures and body image in urogynaecology, and the consequences of obstetric anal sphincter injury. His academic work reflects his subspecialty clinical focus. A full publication list is available on request.

Mr Iles is Chair of the Training Committee of the British Society of Urogynaecology (BSUG) and the BSUG representative on the Subspecialty Training Committee of the Royal College of Obstetricians and Gynaecologists (RCOG). He is Undergraduate Lead for Gynaecology at St Mary’s Hospital. He is the subspecialty training programme supervisor for subspecialty trainees in urogynaecology at Manchester University NHS Foundation Trust, and has held national and international faculty roles in urogynaecology education, including teaching at the RCOG Surgical Masterclass, the BSUG Annual Scientific Meeting and the British and Irish Association of Robotic Gynaecological Surgeons Annual Meeting.

Mr Iles has completed the Cardiff University Bond Solon (CUBS) medico-legal training in report writing, courtroom skills, and civil procedure rules. He is happy to receive instructions from both Claimant and Defendant law firms in personal injury and clinical negligence cases, and is willing to attend court if required. He has a particular interest in cases involving mesh complications and complex pelvic floor surgery.
